Terms & Conditions

Last updated: October 2026

1. Acceptance of Terms

By accessing or using The Rehab Portal platform ("Service"), you agree to be bound by these Terms & Conditions. If you do not agree to these terms, please do not use our Service.

2. Description of Service

The Rehab Portal is a rehabilitation management platform designed for healthcare organizations to deliver pulmonary and cardiac rehabilitation programs. Our Service enables healthcare providers to prescribe exercises, monitor patient progress, and communicate with patients remotely.

3. User Accounts

To access certain features of the Service, you must create an account. You agree to:

  • Provide accurate and complete registration information
  • Maintain the security of your account credentials
  • Notify us immediately of any unauthorized access to your account
  • Accept responsibility for all activities that occur under your account

4. Healthcare Provider Responsibilities

Healthcare providers using our Service agree to:

  • Use the Service in compliance with all applicable laws and professional standards
  • Maintain appropriate licensure and credentials
  • Obtain necessary patient consents before using the Service
  • Make independent clinical judgments regarding patient care
  • Not rely solely on the Service for clinical decision-making

5. Patient Responsibilities

Patients using our Service agree to:

  • Provide accurate health information
  • Follow prescribed rehabilitation programs as directed by their healthcare provider
  • Report any adverse symptoms or concerns to their healthcare provider promptly
  • Understand that the Service is a supplement to, not a replacement for, in-person medical care

6. Medical Disclaimer

The Rehab Portal is a healthcare technology platform and does not provide medical advice, diagnosis, or treatment. The content provided through our Service is for informational purposes only and should not be considered a substitute for professional medical advice.

Always consult with a qualified healthcare provider before starting any exercise or rehabilitation program. If you experience any symptoms or have concerns about your health, contact your healthcare provider immediately.

7. Medication and Symptom Tracking

Medication and symptom tracking features support personal recordkeeping and communication. They do not independently diagnose a condition, prescribe treatment, or recommend medication changes.

You are responsible for checking the information you enter and keeping medication details and reminder settings accurate. Do not start, stop, or change medication solely because of an app reminder, summary, or check-in result. Follow instructions from your qualified healthcare provider.

Reminders, alerts, and reports are not guaranteed to be delivered or reviewed immediately. Using these features does not, by itself, establish a clinician–patient relationship or continuous monitoring.

Do not use the platform to request emergency assistance. If you believe you are experiencing a medical emergency, contact your local emergency services immediately.

8. Intellectual Property

All content, features, and functionality of the Service, including but not limited to text, graphics, logos, and software, are the exclusive property of The Rehab Portal and are protected by copyright, trademark, and other intellectual property laws.

9. Limitation of Liability

To the fullest extent permitted by law, The Rehab Portal shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of the Service. Our total liability shall not exceed the amount paid by you for the Service in the twelve months preceding the claim.

10. Termination

We reserve the right to suspend or terminate your access to the Service at any time for any reason, including violation of these Terms & Conditions. Upon termination, your right to use the Service will immediately cease.

11. Changes to Terms

We may modify these Terms & Conditions at any time. We will notify users of material changes through the Service or via email. Your continued use of the Service after such modifications constitutes acceptance of the updated terms.

12. Governing Law

These Terms & Conditions shall be governed by and construed in accordance with the laws of the jurisdiction in which the subscribing healthcare organization is located, without regard to conflict of law principles.
